Plan Types
Preventive wellness EPO plans designed for individuals in good oral health. Exams, x-rays, and cleanings are covered with no out-of-pocket costs. Basic procedures, such as fillings, are covered at a nominal or no member cost share. Coverage from in-network providers only.
Preventive procedures (exams, x-rays, cleanings) are covered at 100% at in-network providers.4 Fillings, endodontics (root canal), periodontics (gums), and simple extractions are covered. Choose in-network-only access (EPO) or both in- and out-of-network access (PPO).
Coverage includes a combination of preventive, basic, and major restorative procedures, such as crowns, bridges, and implants. Plans typically offer a higher annual maximum. Both in-network-only or in- and out-of-network access is available.
Rich benefit plans that offer generous coverage options for most dental procedures. Members get the lowest out-of-pocket costs and typically the most flexibility in choice. Both in-network-only or in- and out-of-network access is available.
